Telehealth Informed Consent Agreement

Republic of South Africa — Version 1.0 — Last Updated: 26 April 2026

Important Notice

This Telehealth Informed Consent Agreement governs your participation in telehealth services facilitated through the Medora digital health platform. If you do not understand any portion of this consent, seek clarification from your healthcare practitioner before continuing.

1.Nature of Telehealth Services

Telehealth involves the delivery of healthcare services using electronic communication technologies, which may include real-time video consultation, audio communication, secure messaging, electronic clinical documentation systems, and electronic prescribing.

All healthcare services are provided by independent registered healthcare practitioners.

2.No Emergency Services

Important Notice

Telehealth services provided through Medora are not intended for emergency or life-threatening conditions. If you are experiencing severe chest pain, difficulty breathing, loss of consciousness, severe bleeding, seizures, severe symptoms in a child, or rapidly worsening symptoms — you must immediately call 10177 or attend your nearest emergency department.

Telehealth consultations are not a substitute for emergency medical services.

3.Limitations Compared to In-Person Care

You acknowledge and understand that telehealth services have inherent limitations compared to in-person medical examinations, including:

  • •Absence of physical examination
  • •Inability to perform certain diagnostic tests
  • •Limited ability to observe subtle clinical signs
  • •Reliance on self-reported information
  • •Possible incomplete clinical information

4.Risks of Telehealth

You understand that telehealth services involve risks, including but not limited to:

  • •Technical interruptions
  • •Connectivity failures
  • •Audio or visual distortion
  • •Delays in communication
  • •Incomplete transmission of information
  • •Unauthorized access despite reasonable safeguards

5.Accuracy of Information Provided

You acknowledge that clinical decisions depend on the accuracy and completeness of the information you provide. You agree to provide truthful and complete information, disclose relevant symptoms and medical history, and inform the practitioner if symptoms worsen.

Failure to provide accurate information may impact the quality of care provided.

6.Privacy and Confidentiality

Telehealth consultations are conducted using encrypted technologies designed to protect confidentiality. However, you acknowledge that electronic communication carries inherent privacy risks and absolute security cannot be guaranteed.

Your health information will be processed and stored electronically in accordance with applicable South African law, including POPIA.

7.Electronic Prescriptions

You consent to receiving prescriptions electronically where clinically appropriate and permitted by law. The prescribing decision remains solely the professional responsibility of the healthcare practitioner.

8.Electronic Medical Records

You acknowledge and consent that telehealth consultations will be documented electronically. Messages exchanged through the platform may form part of your permanent medical record. Medical records will be retained in accordance with applicable legal and regulatory requirements.

9.Independent Practitioner Acknowledgment

You acknowledge that healthcare practitioners using the Medora platform are independent registered professionals who exercise independent clinical judgment. Medora does not control or direct clinical decisions.

A doctor-patient relationship, if established, exists solely between you and the healthcare practitioner.

10.Right to Withdraw Consent

You have the right to withdraw your consent to telehealth services at any time. Withdrawal may limit your ability to continue receiving care through the Medora platform. Withdrawal does not affect services already provided or documentation already created.

11.Voluntary Participation

Participation in telehealth services is voluntary. You may choose to seek in-person consultation instead. Declining telehealth services will not prevent you from seeking care through other means.
